Note: When giving a date please try to be accurate. You have 3 years from the date of any injury to make a claim otherwise you will be out of time (known as the limitation period). You might be able to rely on a later date of knowledge type argument if you have only just found out that your treatment was substandard- e.g. you have seen another dentist who has recently informed you that your treatment from the previous dentist was substandard.

Note: One of the first jobs that we have to undertake is to obtain copies of your relevant clinical notes from your dentist, and in order to do this, you must supply us with full names and addresses of all of those healthcare professionals that have treated you in the past. By providing this information you are consenting for us to make contact with them
Note: in order to take on your case, I must have carried out a reasonable search for existing legal expenses insurance policies that you may have, sometimes without even realising. Many people have existing legal expenses insurance attached to their home or car insurance, and do not even know about it. This is fairly common. We suggest that you telephone your insurer(s) to ask them whether you have any pre-existing legal expenses insurance before filling in this part of our questionnaire. Please check with your car insurer, house insurer (buildings & Contents), Trade union, Credit or Debit card insurer, Bank Account insurer, Travel insurer. Most insurers do not cover potential dental negligence claims- but you do need to check
